The Pathways Support Services team is driven by the desire to assist NDIS participants to overcome the challenges faced when trying to find somewhere safe to live and where they can receive the highest standard of care and support.

Pathways Support Services is an approved provider under the National Disability Insurance Scheme (NDIS), and our senior team have worked under this model since its inception and, prior to that, the Human Services Quality Framework.
Clients can be confident that our experience ensures we comply with the NDIS Practice Standards and maintain delivery of high-quality services, which are audited each year.
Please note: we are not an aged care or homelessness service. You will require an approved NDIS Plan to access our supports.

Pathways support services stands out from other providers with our truly professional carers working in our Supported Independent Living accommodation on a regular rotation of shifts (based on client’s needs). Our teams in houses typically consist of 4 support workers, allowing for consistency and familiarity of support and continuity of care. This approach is more positive for the client’s care and development. Greater trust is built with carers who positively support social connection with existing and new friendship circles and investment in community involvement. Less staff changes also means less ‘traffic’ and greater home ‘ownership’ by the disabled person.

In addition to our accommodation and specialist support services, Pathways Support Services works with young adults living in vulnerable situations and with low levels of disability to develop the appropriate life skills to transition to independent living. Skills include: cooking; cleaning; basic hygiene; money handling; shopping and more.
