Pathways Support Services provides carefully considered and accessible homes, complete with 24-hour care, so that clients can live proudly and independently and work toward fulfilling their personal goals.
Our homes consider each client’s needs, including those of visiting friends and family, to ensure clients remain connected socially and maintain control of their own lives.
Pathways Support Services aims to assist young people to access age-appropriate housing and to be matched with support workers and housemates (where applicable) that complement their personalities and interests.
If we don’t currently have a home to suit the needs of a participant, or in a desired location, we work with you to identify specific requirements and are able to quickly mobilise new facilities to accommodate their specific requirements.
Continuity of care
Pathways Support Services stands out from other providers with our professional support workers who work in the Supported Living Accommodation on a consistent roster, which is based on the client’s support needs and preferences. We find this approach to be more positive for our clients and their development. Greater trust is built with the support workers who are then able to positively support the client’s social (new and existing friendships) and community connections. A consistent support team and roster also means the clients have less interruption and a greater sense of ownership and control in their home.

Continuity of care
We stand out from other providers with our truly professional carers working in our Supported Independent Living accommodation on a 48-hour roster. This approach is more positive for disabled persons and their development. Greater trust is built with carers who positively support social connection with existing and new friendship circles and investment in community involvement. Less staff changes also means less ‘traffic’ and greater home ‘ownership’ by the disabled person.
